Additional Requirements
Instructors or event facilitators must provide a certificate of insurance and list Embodi Wellness, LLC as the additional insured.
Visitors must sign a liability waiver if utilizing equipment.
The instructor or event facilitator assumes all responsibility for the event they plan/host.
